십대 이전부터 50이 훌쩍넘을 때까지의 매일 매일을 생생히 기억하는 한 여인이 있다.
놀라울만한 기억력에 모두들 놀라고 또 어떤이는 부러워 하기도 하지만
역시 그녀가 말하는것처럼 모두를 기억 하는것은 축복받은게 아닐지도 모르겠다.
좋은 추억을 그때 그대로 기억할수 있는 것처럼 나쁜 일 역시 잊지 못한다면...

우리는 하루 하루를 살아가며, 어떤 추억은 지우려 해도 지울수가 없고 (연애인들의 몸무게라든가)...,
정작 다른 꼭 필요한것들은 아무리 노력해도 절대 외울수가 없다... 해부학 1차 시험공부의 내용이나 아내님의 생일, 아니면 Uma Thurman의 전화번호 같은 것 (바지 주머니에 넣어 놓았다는 사실도 잊곤한다).

기억할수 있는것을 계속 떠올릴수 있고 잊고 앞으로 나아갈수 있는 일을 기억 저편으로 지울수 있다는것은 어쩌면 하나님이 우리에게 주신 축복은 아닐까 생각해본다.

Wallenberg's syndrome

aka "lateral medullary syndrome"

2' to occlusion of PICA or VBA (located laterally to medulla oblongata)
ipsilateral L/O pain & temp of face (trigeminal nucleus)
contralateral L/O pain & temp of body (lateral spinothalamic)
dysphasia (difficulty swallowing & speech 2' to vagus & hypoglossal CN9), decreased gag reflex
if vestibular system involved = pt. may c/o vertigo, N/V, (+) nystagmus
if descending SNS involved = pt. may present c/ (+) ipsilateral Horner's syndrome

Circle of Will

1. ACA = LE>UE
2. MCA = mess, UE>LE, BG - sensory ataxia, (+) Romberg (L/O standing balance c/ eyes closed), speech (wernicke's), HH

3. PCA = Weber's Syndrome (ocular motor palsy c/contralateral hemiplegia/hemiparesis)
4. Basilar a. = brainstem s/s ("locked-in syndrome")
5. PICA = Wallenberg syndrome (ipsi tongue + contra para/paresth.)
6. VBA = Horner's sign (ptosis, miosis, anhydrosis)

ROP (Retinopathy of Prematurity)

Reinopathy of Prematurity (ROP) is a retinal detachment of premature babies. It is 2' to abnormal blood vessel development and was often found c/ pt. who received high O2 dose for RDS or other pulmonary dzs tx. Currently, c/ proper O2 delivery monitoring, risk of ROP due to O2 tx is rare.

Retinal blood vessel development begins at 2nd trimester (30d post-conception); newborns a/ 30 wk gestational age & c/ RDS is in high risk of ROP.

Tx should begin immediately (p/ 72hr) and detachment may require surg. intervention.

few things that I've completely forgotten about...

1) radial nerve innervates all extensors (C6-8) + abductor pollicis longus (AbPL) (C7-8) + supinator (C5-6)

2) adductor pollicis (AdP) is the only thenar muscle innervated by non-median nerve

3) unlike lumbricals, interossei are entirely innervated by ulnar nerve

4) none of the quinti (minimi) muscles are innervated by median nerve

5) Erb's palsy = C5-6, upper arm paralysis including supinator and extensors
Klumpke's palsy = C8-T1, lower arm paralysis including intrinsics
Erb-Klupke's palsy = whole arm paralysis/brachial plexus injury (BPI)
